Initial Visit

Download, print, and complete the patient forms from the link below.
Bring photo ID. Acceptable forms include:
- Valid State Identification Card
- Valid State Driver's License
- School Identification Card
- Valid Passport
Arrive at least 15 minutes before your scheduled appointment to complete necessary paperwork
Wear comfortable clothing
Plan for your evaluation to last 45 minuets to 1 hour and 15 minutes

When calling to set up your initial visit, we will ask for your insurance information. As a courtesy to our patients, we will verify your benefits prior to your initial visit.
